Assisted Living Facilities
Clients residing at Agate Heights ALF are encouraged to assume as much responsibility as possible in the areas of independent living. Clients must be 18 years of age and must be able to independently navigate stairs without assistance. Clients must also be independent in their ability to complete activities of daily living (ADLs), as staff can only remind, encourage, and prompt clients to complete ADLs (including bathing, grooming, eating, toileting, and dressing). The program consists of daily activities, group therapy, individual counseling, and psychiatric/medical services following the rehabilitative model to assist with treatment goals.
Professionally trained Case Managers, Rehab Specialist, Nursing, Program Coordinator, and Mental Health Practitioner offers individualized treatment planning with continued monitoring of each individual's needs and progress. The program approaches treatment holistically, with client-centered and strength-based design by providing the client with a community-like setting in a structured living environment where clients can develop life skills to become self-sufficient. In addition, the program offers the opportunity to create sustainable coping skills, interpersonal skills, med-management skills, behavioral intervention, independent life skills training, and self-awareness of mental health illnesses.

Referrals
Please follow the steps below to submit a referral for Agate Heights Assisted Living Facility
*Note: It is recommended that an individual interested in Agate Heights receive support from a case manager, provider, or family member when completing and submitting the referral.
Once a referral is complete, it is reviewed by the Agate Heights program supervisor to determine financial and clinical eligibility. A referral is considered complete when the referral packet and supporting documentation (detailed below) are sent to Lake Whatcom Center. The program supervisor uses the documentation submitted to assess a client’s current and historical level of functioning, support needs, services received, engagement in services, and response to treatment to ensure the Agate Heights treatment program can adequately support their needs and provide the best course of treatment.
Lake Whatcom Centers (LWC) Alabama House offers assisted living services and specialized care for people managing chronic mental illnesses such as schizophrenia and bipolar who require care, support, and assistance in their daily lives. By providing individualized treatment plans to address health or behavioral changes over time, medication management, and assistance in activities of daily living (ADLs).
