Community Case Management at Lake Whatcom Center supports clients with chronic mental illness to develop skills to manage their symptoms and assists them to access a full range of services in the community. The CCM program works with clients to address areas in life impacted by mental illness by identifying their needs, developing an individualized treatment plan, and monitoring and evaluating progress over time. The program's intensity is continually adjusted to address the full spectrum of treatment, from crisis intervention to independence of case management.

In addition, the Community Case Manager incorporates client-centered care through teaching, modeling, and practicing skills to increase self-sufficiency and independence within various community settings. Including assistance in daily life activities, social and personal growth, financial and legal issues, medication, symptom management, and scheduling and attending medical, dental, and psychiatric appointments. Furthermore, Community Case Managers assist in connecting clients to resources such as referrals to medical/psychiatric/dental care, housing, substance use treatment, therapy, transportation, employment, and so much more!

Community Case Management Services

  • Meetings: once per week to once per month, on average -

  • Medication-assisted/prompting services

  • Assistance with transportation issues, budgeting and maintaining benefits, medical/dental appointments, and activities of daily living.

  • Comprehensive Client screening

  • Licensed professional Clinical and Medical staff trained to treat medical and mental illness

  • Mental health counseling for treatment and relapse prevention

  • Access to Lake Whatcom Center Medical and Psychiatric Services, Therapy, Case Management, Counseling, Housing, and Employment services
