The PACT (Program for Assertive Community Treatment) Program is an effective, evidence-based, recovery-oriented mental health service for individuals with severe and persistent mental illnesses, often with co-occurring substance use disorder. Lake Whatcom Center PACT team serves a caseload of no more than 50 clients that utilizes a trans-disciplinary team approach to providing intensive outreach-oriented services. Services are comprehensive, highly individualized, and modified as needed through an ongoing assessment and treatment planning process based on the needs of the individuals served.
The PACT team consists of a multidisciplinary team of professionals. PACT provides skills and education to clients who experience difficulties with daily living activities, have problems with role functioning, and frequently access high-intensity services. In addition, the program focuses on reducing the individual's need for hospitalization, decreasing their need for crisis services, and improving day-to-day life.
PACT is committed to helping clients increase their quality of life and sustain independence by stabilizing their recovery and meeting their treatment goals in the community.
